About Your Appointment

Before Your Visit

We hope that this will be the first of many visits. In order to have you spend more time with the doctors and less in the waiting room, we have made our patient forms available on our Patient Portal. After you schedule your appointment, please complete these forms and submit electronically through the Patient Portal.

At Your Appointment

Please be sure to bring your most current insurance card and drivers license/photo ID with you to every appointment. Patient's photo identification must be provided to bill your insurance. Notify us if your insurance coverage changes at any time. Please arrive 10 minutes early so your information can be verified.

Appointment Policy

If you are unable to keep your appointment, please call to cancel or reschedule. If you miss your appointment that time cannot be used for another patient. It is our policy to charge $30.00 for missed appointments, unless cancelled at least 24 hours in advance. A new patient who has missed two appointments will not be given any additional appointments and will be billed $30.00 for each missed appointment. An established patient who has missed three or more appointments could be dismissed from the practice.

Payments and Insurance

There will be no refunds allowed for payments made by patients online. Insurance plans vary greatly. For that reason, it is your responsibility to be aware of which medical benefits your insurance company covers. Prior to your appointment, please verify with your insurance carrier which services require referrals or prior authorizations. We participate with most insurance plan/networks. Please confirm with your insurance company that the provider you are scheduled to see is considered "in-network" under your insurance plan. We make every effort to help keep down the cost of your medical care. For this reason we expect payment of any copays or non-covered services at the time of your visit. We accept cash, checks, MasterCard, Visa, Discover or American Express. Answering Service

When the baby is due, call the office first!!!

Usually, the contractions are 5 to 7 minutes apart and painful. 2. CALL: If your membranes rupture (bag of water breaks). 3.
